Your Visit

For new patients: Please contact us to set up an appointment. An EMU Health staff member will follow up with you to schedule your initial appointment and provide access to the patient portal. For subsequent visits, you will be able to use the patient portal.

For existing patients, we offer a patient portal through which you can access your medical records, request new appointments, view upcoming ones, request prescription refills, receive specialty referrals, communicate with our doctor, and much more. To get access, all you need is your pre-assigned login name and password, which can be requested by calling (718) 850-4368, Monday to Friday, 7 am to 5 pm (excluding public holidays).

EMU Health is licensed by The New York State Department of Health under Article 28 of the New York State Public Health Law.

The EMU Specialty Center incorporates a comprehensive Quality Assurance program, overseen by a Quality Assurance/Continuing Quality Improvement (CQI) Committee under the guidance of EMU Health’s medical director. Additionally, EMU Health’s management and staff work attentively to enforce all HIPAA compliance guidelines.

We are located in Glendale, Queens, which is near Middle Village (11379), Elmhurst (11373), Ozone Park (11416), Jamaica (11435), Maspeth (11378), Kew Gardens (11415), Woodhaven (11421), Rego Park (11374), Forest Hills (11375), Ridgewood (11385), and Richmond Hill (11418).
