Referral Process

If you would like to refer a patient to EMU Health, you have a few options:

  1. You can submit a patient referral request by filling out the form below. Just give us a few pieces of information and we will call you to discuss your patient and referral needs.
  2. Your office or your patient can call (718) 850-4368 and a scheduler will set your patient up at the clinic.
